2017 AHA/ACC/HRS Guideline for Management of Patients With Ventricular Arrhythmias and the Prevention of Sudden Cardiac Death

The US guideline on ventricular arrhythmias appeared a year after DANISH and read that trial very differently from the Europeans: it keeps the primary-prevention ICD at Class 1 for symptomatic heart failure with LVEF ≤35% on optimal therapy without distinguishing ischaemic from non-ischaemic aetiology, whereas the ESC 2022 guideline downgraded non-ischaemic dilated cardiomyopathy to Class IIa on the same evidence. That single divergence still explains most differences in ICD implantation rates between the two regions. Beyond devices, the guideline stresses that the therapy which prevents most sudden deaths is guideline-directed medical heart-failure therapy, and it covers catheter ablation of ventricular tachycardia and the disease-specific channelopathy pathways.
