In 518 patients with complicated intra-abdominal infection after adequate source control, a fixed 4-day antibiotic course was equivalent to therapy until 2 days after resolution of physiological signs (~8 days) for surgical-site infection, recurrent IAI or death.

After source control, 4 days suffices for complicated IAI — a foundational shorter-is-better trial.
