IRIS-MAIN

IRIS-MAIN: Long-Term Survival after Coronary Revascularization for Unprotected Left Main Disease, a Prospective, Multicenter, Multinational Cohort Study

Patient / Population Intervention / Exposition Vergleich Endpunkt

In patients with unprotected left main coronary artery disease, PCI with drug-eluting stents showed no statistically detectable difference in 10-year all-cause mortality compared with CABG.

Design
Prospective, multicentre, multinational (Korea and the Asia-Pacific region) observational registry of unprotected left main disease treated with PCI, CABG or medical therapy, enrolling since 2007
Endpunkt
Death (10-year follow-up)
Ergebnis10-year weighted Kaplan-Meier all-cause mortality with PCI 24.0% vs 22.9% with CABG, overlap-weighted hazard ratio 1.00 (95% CI 0.87-1.14; p=0.99), with no statistically detectable difference between PCI and CABG. Median follow-up was 10.0 years (IQR 5.0-16.7).

Diskussion & Kritik

This large registry with a median follow-up of 10 years found no detectable difference in all-cause mortality between PCI with drug-eluting stents and CABG for left main disease. It is non-randomised, and although overlap weighting within a target trial emulation framework was used, the authors state that the findings cannot establish causal treatment effects or equivalence in survival. The confidence interval does not exclude a mortality benefit or harm with either strategy. The abstract reports no figures for myocardial infarction, stroke or repeat revascularisation, so the trade-offs beyond survival cannot be judged from it.
