GBD degenerative mitral valve disease

Global Burden and Outcomes of Degenerative Mitral Valve Disease: A 30-Year Analysis of the Global Burden of Disease Study

Patient / Population Intervention / Exposure Comparison Outcome

In the global population, degenerative mitral valve disease showed a 126% rise in prevalence and a 75% rise in deaths over three decades, while age-standardised prevalence stayed mostly stable.

Design
Analysis of 30 years of Global Burden of Disease Study data on degenerative mitral valve disease
Endpoint
Prevalence, deaths and disability over 30 years
ResultPrevalence of degenerative mitral valve disease increased by 126% over the three decades, with an estimated 16.1 million individuals affected. Age-standardised prevalence was mostly stable over time (175.4 per 100,000). DMVD-related mortality rose by 75% to almost 40,000 deaths annually, with the greatest number of deaths between ages 85 and 89. The disease was more commonly identified in men than in women (2.39 vs 1.29 million), but women more commonly died from it (24,400 vs 15,400). Global disability-adjusted life years lost due to DMVD increased by 66% since 1990.

Discussion & critique

This analysis shows that the absolute burden of degenerative mitral valve disease has grown over three decades while age-standardised prevalence stayed mostly stable, which points to population ageing rather than a rising rate. It is a modelled epidemiological estimate, not a trial, so it tests no intervention and cannot establish cause. Cases depend on echocardiography or ICD coding, so underdiagnosis and survival bias in low-resource settings probably understate true prevalence, as the presenters acknowledged. The report gives no uncertainty intervals and no full methods, and the findings have not been peer reviewed.
