DUO-E
Durvalumab (D) plus carboplatin/paclitaxel (CP) followed by D with or without olaparib (O) in advanced/recurrent endometrial cancer (a/rEC): Interim overall survival (OS) and safety from the randomised phase 3 DUO-E/GOG-3041/ENGOT-EN10 study
Patient / Population Intervention / Exposure Comparison Outcome
In patients with first-line advanced or recurrent endometrial cancer, carboplatin/paclitaxel plus durvalumab followed by durvalumab with or without olaparib showed a favourable but not statistically significant trend in overall survival at an interim analysis compared with carboplatin/paclitaxel alone.
Design
Randomised phase 3 trial (NCT04269200) in first-line advanced/recurrent endometrial cancer, with patients randomised 1:1:1 to carboplatin/paclitaxel (CP; 241 pts), CP plus durvalumab followed by durvalumab (CP+D; 238 pts) or CP plus durvalumab followed by durvalumab plus olaparib (CP+D+O; 239 pts), blinding not stated in the abstract.
Endpoint
Dual primary endpoints of progression-free survival in the intention-to-treat population for CP+D vs CP and CP+D+O vs CP. This report is the prespecified interim analysis of overall survival, a secondary endpoint.
ResultAt the interim OS analysis (52% maturity; median follow-up 52 months in censored pts), median OS in the ITT population was 39.6 months (95% CI 30.3-49.2) with CP, 40.7 months (95% CI 34.7-57.9) with CP+D and 51.1 months (95% CI 44.2-NR) with CP+D+O. HR vs CP 0.87 (95% CI 0.68-1.11; P = 0.259) for CP+D and 0.77 (95% CI 0.60-0.98; P = 0.037) for CP+D+O, reported as a favourable though not statistically significant trend. OS events 136/241 (56.4%) vs 123/238 (51.7%) vs 115/239 (48.7%). In dMMR pts, median OS 50.2 months (95% CI 17.3-NR) with CP vs NR with CP+D and NR with CP+D+O; HR 0.57 (95% CI 0.30-1.05) for CP+D and 0.43 (95% CI 0.21-0.83) for CP+D+O. In pMMR pts, median OS 37.8 months (95% CI 27.0-48.6) vs 38.3 months (95% CI 26.3-54.5) vs 45.8 months (95% CI 37.1-58.7); HR 0.95 (95% CI 0.73-1.24) for CP+D and 0.85 (95% CI 0.65-1.11) for CP+D+O. The authors report an improvement in median OS with CP+D+O vs CP of 11.5 months (ITT) and 8.0 months (pMMR). Consistent benefits in TFST, TSST and TDT vs CP were reported (ITT). Safety remained manageable and generally consistent with the individual components, with no new concerns.
