African-American Heart Failure Trial

In 1,050 Black patients with NYHA III-IV HFrEF on standard therapy, fixed-dose isosorbide dinitrate plus hydralazine improved a composite of survival, HF hospitalisation and quality of life compared with placebo.

N
1.050
Design
Double-blind placebo-controlled RCT, stopped early for benefit
Endpunkt
Weighted composite score (all-cause death, first HF hospitalisation, change in quality of life)
Relevanz
3
ErgebnisComposite score significantly favoured active therapy; trial halted early for mortality benefit (6.2% vs 10.2% deaths; HR 0.57, P=0.01) and fewer first HF hospitalisations (16.4% vs 22.4%).

Diskussion & Kritik

Enrolled only self-identified Black patients already on ACEi/beta-blockers, limiting generalisability and raising questions about race-based prescribing. Established fixed-dose isosorbide dinitrate/hydralazine as add-on therapy.
