European Respiratory Society clinical practice guideline for the management of adult bronchiectasis
Eight PICO questions and three narrative ones, for a disease the task force calls historically neglected and treated very differently from country to country. The strong recommendations are not drugs: airway clearance techniques for most patients, and pulmonary rehabilitation for those whose exercise capacity is impaired. Two strong recommendations do concern antibiotics: long-term macrolides for patients at high risk of exacerbation, and long-term inhaled antibiotics for those chronically infected with Pseudomonas aeruginosa and at high risk of exacerbation. Eradication treatment and mucoactive drugs are supported conditionally, in defined circumstances. Two things are suggested against: long-term oral non-macrolide antibiotics, and inhaled corticosteroids as routine. The register's entry below is the trial of a neutrophil serine protease inhibitor, the first drug class developed for bronchiectasis itself rather than borrowed from cystic fibrosis.
