KDIGO 2024 Clinical Practice Guideline for the Management of Lupus Nephritis

The 2024 lupus-nephritis guideline replaces the corresponding chapter of the 2021 glomerular-diseases guideline. Its central shift is from monotherapy induction to routine triple therapy: glucocorticoids plus mycophenolate or low-dose cyclophosphamide, with belimumab or a calcineurin inhibitor (voclosporin) added upfront in class III/IV disease. Glucocorticoid exposure is cut sharply, treatment duration is defined by complete renal response rather than a fixed calendar, and repeat biopsy is given a clearer role.
