KDIGO 2022 Clinical Practice Guideline for Diabetes Management in Chronic Kidney Disease

The 2022 update sets out a layered treatment approach for type-2 diabetes with CKD: RAS blockade plus an SGLT2 inhibitor as the foundation, metformin where eGFR allows, a nonsteroidal MRA (finerenone) for residual albuminuric risk, and a GLP-1 receptor agonist where glycaemic targets are unmet or cardiovascular risk is high. It also reworks glycaemic monitoring in advanced CKD, where HbA1c becomes unreliable. A focused update — the KDIGO 2026 diabetes-in-CKD guideline, reworking the chapters on risk assessment, glycaemic monitoring and comprehensive pharmacotherapy — completed public review in April 2026 and is being finalised; until it is published the 2022 text remains in force.
