2026 ACC/AHA/AACVPR/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA Guideline on the Management of Dyslipidemia
The 2026 multisociety guideline retires the 2018 Blood Cholesterol guideline and moves US lipid management closer to — but not onto — the European model. Primary prevention stays risk-based: 10-year risk is now estimated with the PREVENT-ASCVD equations (a 'calculate–personalise–possibly reclassify' pathway in which coronary artery calcium resolves the borderline and intermediate strata), and statin intensity follows the risk category rather than a numeric goal. What is new is the secondary-prevention arm, which for the first time names explicit goals — LDL-C <55 mg/dL (1.4 mmol/L) and non-HDL-C <85 mg/dL in very-high-risk atherosclerotic disease — which is where the ESC/EAS 2019 guideline with its 2025 Focused Update has stood for years. Lipoprotein(a) should now be measured at least once in every adult, and apoB is endorsed for residual-risk assessment.
