2025 AHA/ACC/AANP/AAPA/ABC/ACCP/ACPM/AGS/AMA/ASPC/NMA/PCNA/SGIM Guideline for the Prevention, Detection, Evaluation and Management of High Blood Pressure in Adults
The 2025 guideline replaces the 2017 document and keeps the defining American position: hypertension begins at 130/80 mmHg (stage 1), where the ESC 2024 guideline still reserves the diagnosis for ≥140/90 mmHg and calls 120–139/70–89 mmHg 'elevated blood pressure'. The two bodies have nevertheless converged on treatment: ACC/AHA target <130/80 mmHg for essentially everyone and encourage <120/80 mmHg where tolerated, against the ESC's on-treatment systolic band of 120–129 mmHg. Drug therapy is now triggered at ≥130/80 mmHg in established cardiovascular disease, diabetes, chronic kidney disease or a PREVENT 10-year risk ≥7.5%, and stage 2 hypertension starts on two agents in a single pill.
