2024 AHA/ACC/AMSSM/HRS/PACES/SCMR Guideline for the Management of Hypertrophic Cardiomyopathy

The dedicated US HCM guideline — the ESC covers HCM inside its 2023 cardiomyopathy document — and it was reaffirmed as current in May 2025. It gives cardiac myosin inhibitors a formal place in symptomatic obstructive HCM on the strength of EXPLORER-HCM and VALOR-HCM, and it loosens the old blanket restriction on vigorous exercise, which now becomes a shared decision rather than a prohibition. The most persistent transatlantic difference is sudden-death risk stratification: the ACC/AHA works from a set of individual major risk markers plus imaging (apical aneurysm, extensive late gadolinium enhancement), while the ESC calculates the HCM Risk-SCD five-year probability — the two approaches select overlapping but not identical patients for an ICD.
