2023 ACC/AHA/ACCP/HRS Guideline for the Diagnosis and Management of Atrial Fibrillation
The US guideline restructures atrial fibrillation as a disease continuum in four stages (at risk, pre-AF, AF, permanent AF) rather than by episode duration, and it anchors the decision to anticoagulate on an estimated annual thromboembolic risk of ≥2% rather than on a fixed score cut-off, allowing scores other than CHA₂DS₂-VASc. The ESC 2024 guideline arrives at a similar place from another direction, dropping sex from the score entirely (CHA₂DS₂-VA). The sharper difference is the left atrial appendage occluder, which the ACC/AHA upgraded to Class 2a for patients with a non-reversible contraindication to long-term anticoagulation, while the ESC keeps it at IIb.
