2015 ACC/AHA/HRS Guideline for the Management of Adult Patients With Supraventricular Tachycardia

The US SVT guideline predates the ESC's by four years and the two are largely concordant: vagal manoeuvres and adenosine first for acute termination, catheter ablation as definitive therapy for symptomatic AVNRT and AVRT, and electrophysiological risk stratification rather than reflex ablation in asymptomatic pre-excitation. Randomised evidence in this field is unusually thin, so the recommendations lean heavily on registries and small crossover studies — the three register trials below carry a disproportionate share. Where the documents differ it is mostly a matter of vintage: the modified Valsalva manoeuvre (REVERT) landed in the same year as this guideline and only reached full guideline status with the ESC in 2019, which is also more assertive about ablating high-risk asymptomatic pre-excitation.
